Prima facie cause of the accident: “While two Work persons were removing the wheel nuts from a tyre mounted on Dumper which was gripped by tyre handler, Rim came out hitting the persons removing the bolts causing fatal injuries to both.” Wheel Rim came out of the tyre and hit the persons removing the bolts.

Persons were removing the Wheel Nuts staying between the Jaws of Tyre Handler
